The DOT medical card for FedEx Ground drivers, explained

Why FedEx Ground drivers need a DOT medical card, what the physical checks, where to get it and how it is verified through First Advantage.

Why non-CDL drivers still need it

FedEx Ground requires its contractors' drivers to hold a valid DOT medical certificate regardless of vehicle size. Contractors cannot put you on a route without it, and First Advantage verifies it as part of processing.

What the exam checks

Vision (20/40 in each eye, with correction allowed), hearing, blood pressure (under 140/90 passes for two years), urinalysis for protein and sugar, and a review of medications and conditions such as sleep apnea or diabetes. Most exams take 30 minutes.

What to bring

Photo ID, glasses or hearing aids if you use them, a list of medications, and any specialist letters for a condition you manage. If you take blood pressure medication, bring it and take it that day.

Where and how much

Any certified medical examiner on the FMCSA registry; occupational clinics and many urgent cares qualify. Expect $75 to $150. Contractors we recruit for schedule the physical together with the drug screen so it is one trip.

How long it lasts

Two years for a clean exam; one year or three months when the examiner wants a condition rechecked. Put the renewal date on your phone; an expired card stops you from driving the next morning.

Do Amazon DSP drivers need a DOT card?

Not for standard delivery vans. Amazon DSPs use their own screening provider and do not require the DOT medical certificate.

Need road-test-ready drivers?

Plans from $399 a month for Amazon DSPs and $449 for FedEx Ground ISPs, with a guaranteed minimum of candidates.
